You could ask the person who gave you the gift if it is cotton or some other
fabric "so that you will know when it needs to be cleaned." That would
avoid the need to mention that the afghan was soiled before it arrived, but
you would know more about appropriate cleaning methods.

That's a good idea. I crochet, and when I make a larger item I'll keep
one of the yarn wrappers and tuck that into the gift, so the recipient
has the washing instructions.
