My experience has been that, with potentially-cardiac problems, the ER staff get you hooked up to the EKG first, ask about symptoms, and then, once they are fairly sure that you aren't about to drop dead on the spot, have someone come in with a clipboard to fill out the paperwork. If you have a non-life-threatening condition, such as a broken bone, they are more likely to do the paperwork as the first step. -- John F.
